Introduction: More Than Just Eggs and Sperm
When you think about fertility struggles, you probably think about egg quality, sperm count, or blocked tubes. But what if the obstacle is invisible?
For a growing number of couples, the root cause of infertility isn’t structural—it’s immunological. In fact, immunological factors in fertility are now recognized as a leading cause of recurrent implantation failure (RIF).
At Medical Art Center, we don’t just look at the anatomy. We investigate the immune system to answer one crucial question: Why isn’t the embryo staying?
The Paradox: A Helper and a Hurdle
Your immune system is designed to protect you. It attacks viruses, bacteria, and foreign cells. But an embryo is unique—it is 50% foreign (from the father).
Normally, your body performs a miracle. It suppresses the aggressive immune cells specifically in the uterus to “tolerate” the embryo. This is called immune tolerance.
The Problem: For some women, this suppression doesn’t happen. The immune system mistakes the embryo for a threat (like a virus) and attacks it before it can implant.
3 Key Immunological Factors Affecting Implantation
If you have experienced multiple failed IVF cycles with good-quality embryos, one of these three factors could be the culprit.
1. Natural Killer (NK) Cells – The “Assassins.”
NK cells are part of your innate immunity. In the blood, they fight cancer. In the uterus (uNK), they help remodel blood vessels. However, when activated too aggressively, they release chemicals (cytokines) that are toxic to an embryo.
- High uterine NK cells → Increased embryo rejection.
2. Th1/Th2 Ratio – The Balance
Think of T-helper cells as the generals of your immune army.
- Th1 are aggressive (inflammation).
- Th2 are protective (pregnancy-friendly).
If your Th1 cells dominate (high Th1/Th2 ratio), your body creates a “pro-inflammatory” environment. An embryo cannot survive in an inflamed uterus.
3. Antiphospholipid Antibodies (APA)
These are autoantibodies that attack the fats (phospholipids) in cell membranes. They cause microscopic blood clots in the uterine lining. Even if the embryo attaches, the blood supply is cut off, leading to very early miscarriages (chemical pregnancies).
Signs You Might Have an Immune Issue
You don’t need to guess. However, you should ask your doctor about immunological testing if you have:
- 3 or more failed IVF cycles with good embryos.
- A history of recurrent miscarriages (before 10 weeks).
- A known autoimmune disease (Lupus, Rheumatoid Arthritis, Hashimoto’s Thyroiditis).
- Endometriosis or unexplained infertility.
How Medical Art Center Diagnoses Immune Infertility
Standard fertility clinics stop at the ultrasound. We go deeper. At Medical Art Center, our Reproductive Immunology protocol includes:
- Endometrial Biopsy (ERA + EMMA/ALICE): We check for chronic inflammation (endometritis) and NK cell activity directly in the uterine lining.
- Peripheral Blood NK Cell Assay: Measures the killing capacity of your immune cells.
- Th1/Th2 Cytokine Panel: Quantifies the inflammatory ratio.
- APA & Thrombophilia Panel: Checks for clotting factors.
Treatment: Calming the Immune Storm
The good news? An overactive immune system can be managed. If we find an imbalance, we don’t guess—we tailor a protocol.
Common Immunological Treatments:
- Intralipid Infusions: These fatty acids help calm NK cell activity. Unlike steroids, they have fewer side effects.
- Low-Dose Steroids (Prednisolone): Suppresses the general immune attack during the implantation window.
- Intralipids vs. IVIG: We use Intralipids as a safer, more cost-effective first-line therapy.
- Blood Thinners (Lovenox/ Heparin): For APA or clotting issues.
- Tacrolimus (FK506): For extremely high NK cell cases (used off-label with strict monitoring).
Important: Never take these medications without testing. Wrongly suppressing the immune system can lead to infection or cancer. Always treat based on lab data.
A Note on “Receptivity”
Immunology is the final frontier in fertility. You can have a perfect blastocyst (genetically normal) and a perfect uterine lining (thick trilaminar), but if the immune dialogue is wrong, pregnancy won’t happen.
That is why the Medical Art Center unites embryology with immunology. We treat the whole person, not just the cycle day.
Take the First Step
If you are tired of negative pregnancy tests with no explanation, it is time to look inside your immune system.
Don’t let your body reject your dream.
